Step-by-Step: Your First Appointment

  • Scheduling as a New Patient

    Use the appointment scheduler or chat bot on our website to request your initial visit.

    You will have to fill out a pre-screening form that will help gather your personal information, insurance details, and payment method.

    Once your appointment is confirmed, you’ll receive an email inviting you to activate your patient portal where you will complete your new patient paperwork.

    Note: If you have insurance, we will verify it before confirming the appointment.

  • Activate Your Patient Portal

    Check your email for an invitation to create your patient portal account with Charm Health. This is where you’ll complete all intake forms, request refills, fill out questionnaires, communicate with your provider, and manage your care.

    You can also access the portal anytime by clicking the Patient Portal button on our website or by downloading the Charm mPHR app on your smartphone.

  • Complete Intake Forms

    After your appointment is confirmed and you activate your portal, fill out the required questionnaire intake and consent forms and upload pictures of the front and back of your insurance card under Documents.

    Please complete all forms within 48 hours of receiving them to avoid delays or rescheduling.

    Completing your forms early helps us review your history in advance and maximize time focused on your needs.

  • Day of Appointment

    Before your scheduled time:

    Make sure you’ve downloaded the Zoom app on your device (no Zoom account is required)

    Gather your vital signs (blood pressure, heart rate, and temperature) to share with your provider

    Click the video link in the reminder text or email to join your secure Zoom session a few minutes early. Your provider will meet you there to begin your visit.

    (Cash-pay patients: Payment will be automatically processed 24 hours before your scheduled appointment. If your card declines, you’ll be notified to update it before your visit.)
